Helping You Understand Your Options
Advance Care Planning is vital for everyone – patients, their families, health care providers and the community at large – to clearly direct each person’s very personal choices for future medical care, including end-of-life decisions. This is especially important should you no longer be able to speak or advocate for yourself.
Members of our staff can help with information about the primary documents to be completed as you plan, in advance, for future health care choices. Examples of documents we recommend include the Living Will and Durable Power of Attorney for Health Care.
The Foundation for Healthy Communities is an excellent resource for all these topics.
