Caring For You Where You’re Comfortable
If you or a loved one have had a recent medical issue, are struggling to manage a chronic disease, have been recently hospitalized, are having difficulty managing medications, have experienced a fall or are concerned about strength and balance, you would likely benefit from home health care services. In coordination with your physician, home health care services are designed to help people of all ages better manage their health and medications, get stronger, return to a level of independence and stay in their homes for as long as possible. For those who qualify, there is coverage through Medicare, Medicaid and many commercial insurances.
Nursing
Assistance with medication management, wound care, education on managing your chronic medical conditions, IV therapy, communicating with your doctors and other services as needed.
Rehabilitation Services
Physical Therapy, Occupational Therapy and Speech Therapy to help you gain strength, balance, function and endurance in order to improve your safety at home.
Home Health Aides
Providing personal care assistance, if appropriate.
Social Work
Identifying and connecting you to other supportive services that might benefit you and your family.
